4. SMS Terms and Conditions
By opting in, you consent to receive SMS communications from Elite Pain & Health related to customer care, appointments, notifications, and one-time verification codes (OTP) used to confirm your identity when accessing our patient forms or returning-patient follow-up portal. Message frequency may vary depending on your interaction with us. Standard carrier message and data rates may apply based on your mobile plan. Your personal identifiable information, including your SMS consent, will not be shared or sold to any third party or affiliate for any marketing purposes. How to opt in
You may opt in to receive SMS communications by checking the SMS consent box on any of our intake or contact forms, or by providing your mobile number directly to our staff and confirming your consent. The SMS consent checkbox is optional and is not required to submit any form or to receive care.
About one-time verification codes (OTP)
One-time verification codes are sent only in response to a login or form-access request that you initiate yourself. Elite Pain & Health does not send unsolicited verification codes. Codes are time-limited and may be used only once. If you receive a verification code that you did not request, do not share it with anyone, and contact our office promptly.
